VACATION
Eligibility for vacation begins after the successful completion of the initial 150,000 miles.
MEDICAL INSURANCE
Southwest Freight Lines pays 75% of the costs of these benefits and employee pays 25% of the cost. (Pre-tax). Details of Health Plan will be provided in advance of enrollment to eligible employees. Eligibility follows completion of three months of employment.
DENTAL INSURANCE (OPTIONAL)
Paid by employee according to schedule of benefits. Plans starts at $9.95 per month.
LIFE INSURANCE
Coverage of $15,000, provided at no cost to employee.
Supplemental coverage available with contribution by the employee.
Employees are eligible following completion of three months of employment.
ACCIDENTAL DEATH AND DISMEMBERMENT (AD&D)
Insurance coverage is provided at no cost to employee. Provides protection in case of serious injury or death resulting from an accident.
Eligibility follows completion of three months of employment
Life Insurance and AD&D requires participation in the Medical Insurance Benefit Program Plan.
LONG TERM DISABILITY PLAN
Provides an income if you become disabled.
Available with contribution by the employee.
TAX DEFERRED EMPLOYEE SAVINGS PLAN 401K
Supplements Social Security with personal and Company matched savings.
Eligibility: Full time employees after 0ne year of service and 21 years of age, (or 1000 hours in the first fiscal year from the hire date or any subsequent calendar year, as a part time employee).
Employee Contributions: Employees may contribute up to 15% of their salary up to $10,000 in tax saver contributions per year. Special legal limitations apply to highly compensated employees.
Company Contributions: Southwest Freight Lines provides a match up to 4% (to tax saver) contributed by
employee, base match of $.25 per dollar. Investment Options: 12 funds to choose from. You can invest in any of the options in any amount; however, it must be in 10% increments.
